To ask the Secretary of State for Health, what progress the Government has made on integrating and improving care provided outside of hospitals.

Through the £5.3 billion Better Care Fund local authorities and the National Health Service are using pooled budgets to deliver integrated care for their local populations.
The NHS is working with 29 vanguard sites across the country as part of the New Care Models programme to support the improvement and integration of services, backed by a £200 million Transformation Fund.

Secretary of State was originally the title given to the two officials who conducted the Royal Correspondence under Elizabeth I. Now it is the title held by some of the more important Government Ministers, for example the Secretary of State for Foreign Affairs.
