To ask the Secretary of State for Health, what the rate of admissions to hospital with an (a) primary and (b) secondary diagnosis of cardiovascular disease per 100,000 population as recorded by the Hospital Episodes Statistics database was in (i) England and (ii) each local commissioning organisation area in each of the last 10 years.
