The Abortion Regulations 1991 require that registered medical practitioners notify the chief medical officer on form HSA4 of every abortion they have performed. The information required to be completed on the HSA4 form is set out in the regulations. There are no plans to amend the regulations or the HSA4 form to collect information on whether one or both of the certifying doctors have been challenged about their decision that there are grounds for an abortion.
