We are aware of three cases in 2012 where a tag was initially fitted to the wrong person. In each instance, the investigation report revealed that there had been a deliberate attempt to deceive the field officer responsible for fitting the tag. Immediate follow-up action was taken to ensure that the correct individuals were dealt with appropriately.
