To ask the Secretary of State for Health how many patients representing what proportion of admissions died in (a) intensive care units, (b) theatre and (c) other wards in each NHS hospital in each of the last 10 years; and how many and what proportion of people attending accident and emergency departments died while in the department in each accident and emergency department in each such year.
