To ask the Secretary of State for Health how many out of areas referrals were made by the Gateshead and South Tyneside Primary Care Trust because the surgery options sought by the patient were provided by a neighbouring primary care trust rather than Gateshead Primary Care Trust in (a) 2005, (b) 2006, (c) 2007 and (d) 2008 to date.
