To ask the Secretary of State for Health what (a) discussions he has had and (b) representations he has received on streamlining the IRI form to ensure all NHS employees are able to document all incidents in hospitals.

The Department has had no discussions, and is unaware of receiving any representations, about streamlining the IRI form, the name many national health service organisations give to their locally produced incident report form. NHS staff and service providers are also encouraged to share these reports of patient safety incidents with the National Patient Safety Agency using the National Reporting and Learning System.
