To ask the Scottish Executive how many NHS boards have developed models of practice to ensure that every patient receives their medicines, and has any medication issues, addressed in advance of discharge from hospital, as set out in The Right Medicine: A Strategy for Pharmaceutical Care in Scotland .

A practice model has been developed together with a capacity plan to enable clinical pharmacy services identify the resources and systems of work required to achieve the practice model standard. The practice model and capacity plan are currently being validated by practitioners before issue to NHSScotland for local implementation.
