To avoid people being misled, let me say that our policy is clear: we accept the current payment by results tariff. However, as I have made clear, what we want to do is to allow that tariff progressively to become not just a uniform price, but a maximum price against which discounts are available. A GP budget holder exercising the sort of powers that my right hon. and learned Friend Mr. Clarke mentioned would be free to purchase at the tariff, which is simple, or to negotiate discounts, or to negotiate improvements in quality.
